Often times patients walk in with pain in the buttock traveling down the leg and the common misconception is that of a disc bulge or herniation causing sciatica.  However, in some cases the sciatic nerve may be compressed by the piriformis muscle. The problem is easily evaluated by resisted external rotation  of the hip.   Also pressing directly on the piriformis muscle, may cause a referred pattern down the back of the leg.

Treatment usually involves Active Release Techniques, postisometric relaxation techniques, stretching.  We might also suggest  using orthotics in your footwear, as having a short leg, pelvic rotation or foot pronation is a  predisposition for piriformis symdrome.
