Your privacy matters to us. This policy explains what personal and health information we collect, how we use it, how we protect it, and the control you have. We follow Ontario privacy law and the rules of our regulatory college.

Protecting your personal and health information is fundamental to the trust you place in Active Chiropractic. This Privacy Policy explains what information we collect, why we collect it, how we use it, and the choices you have. We comply with the Personal Information Protection and Electronic Documents Act (PIPEDA) and the privacy bylaws of the College of Chiropractors of Ontario.

We collect only the information needed to provide your care and run our practice. This may include:
- Contact details — name, address, phone number, and email address.
- Health information — your health history, symptoms, and concerns you share through our online questionnaire, intake forms, or in person.
- Insurance details — your insurer and policy information, only as needed to direct-bill on your behalf.
- Technical data — basic, non-identifying analytics about how visitors use our website, such as pages visited.
3. How We Use Your Information
We use your information to provide chiropractic and related care, schedule and manage your appointments, communicate with you about your treatment, process insurance billing, maintain your clinical records as required by our regulatory college, and improve our website and services. If you fill out our online assessment questionnaire, the information you provide helps us understand which service may help you and prepare for your visit — it is not a diagnosis and does not replace an in-person assessment.
4. How We Share Your Information
We do not sell your personal information. We share it only as necessary to provide your care and operate the Clinic:
- With your insurer, to direct-bill your extended health care plan with your consent.
- With our booking and electronic health record provider (Juvonno) to schedule appointments and maintain records.
- With your referring physician or other treating providers, only with your consent or as required by law.
- As required by law, court order, or regulation, or to protect the rights, safety, or property of our patients and staff.
5. How We Protect Your Information
We maintain administrative, technical, and physical safeguards designed to protect your personal and health information against unauthorized access, loss, or misuse. Your clinical records are stored securely within our electronic health record system, and access is restricted to authorized Clinic staff who need the information to provide your care.
6. Cookies & Website Analytics
Our website may use cookies and similar technologies to remember your preferences and to understand, in aggregate, how visitors use the site. We use privacy-conscious, anonymized analytics and do not track you across other websites for advertising. You can control or disable cookies through your browser settings; some site features may not work properly if cookies are disabled.
7. Retention of Your Information
We retain your clinical records for the period required by the College of Chiropractors of Ontario and applicable Ontario law. Once the retention period expires, we securely destroy or anonymize the information. Non-clinical contact details (such as email sign-ups) are retained only as long as needed for the purpose collected.
8. Your Rights
You have the right to access your personal information, understand how it is used, request corrections if it is inaccurate, and, in certain circumstances, ask that it be withdrawn or deleted. To exercise any of these rights, or to ask a question about your privacy, contact us using the details below. We will respond in accordance with applicable law.
9. Third-Party Services
When you book online, your information is handled by our third-party booking provider according to its own privacy policy. Similarly, submitting an online assessment passes your details to us through our secure tracking system. We encourage you to review the privacy practices of any third-party service you link out to from our website.
10. Changes to This Policy
We may update this Privacy Policy to reflect changes in our practices or the law. The "last updated" date at the top of this page shows when it was last revised. We will not materially reduce your privacy protections without first notifying you where practicable.
